The registration process is as follows:

- ✅ Confirm Participant Information
- 📌 Review & SUBMIT

(Emailed to your parent/guardian to read and sign once you complete the steps above) - 📬 CONFIRMATION
(Emailed to you to confirm receipt of your registration)
You are currently on step 3 of the registration process, which is to Review & Submit your registration tasks in SurveyMonkey Apply To do this, follow the instructions outlined below:
- Log in to your SurveyMonkey Apply account using the email address you used to create your application.
- Click Review on the left-hand side of your tasks list
- Select “SUBMIT YOUR APPLICATION” located at the top of the review page.

Select “SUBMIT” again in the confirmation window
- You will then be directed to the Submission Confirmation page and will receive an email confirmation that DocuSign forms have been sent to your parent/guardian’s email for signature.
